<div class="content"><span style="padding-left: 50px;">(COLUMBIA, MO)</span> - <em>Correction: The previous press release contained an incorrect link to the survey. The correct link is <a href="https://beheard.como.gov/live-well-boone-county">https://beheard.como.gov/live-well-boone-county</a>. The amended press release is below.</em><br /><br />Columbia/Boone County Public Health and Human Services (PHHS), in collaboration with Central Missouri Community Action (CMCA), asks the Boone County community to participate in the 2023 Live Well Boone County Community Health Assessment survey.<br /><br />This survey seeks to better understand community health and health factors. Results from this survey will inform local funding decisions and public health programming. In the last five years, nearly $42 million has been invested in health and social services based on the results from the previous community health survey. These programs served over 415,000 total people in Boone C
 ounty.<br /><br />"We want to listen to the health needs that people are seeing in our community," PHHS Assistant Director Rebecca Roesslet said. "The input of our community members is invaluable, and we encourage everyone to share their thoughts so we may have the most accurate picture of health in Boone County."<br /><br />Boone County residents can provide input through an online survey found at <a href="https://beheard.como.gov/live-well-boone-county">https://beheard.como.gov/live-well-boone-county</a> or via hard copy surveys which can be found at several locations:<br /><br />Public Health and Human Services, 1005 W. Worley St.<br />City Hall, 701 East Broadway<br />Daniel Boone Regional Library, 100 W. Broadway.<br />Boone County Government Center, 801 E. Walnut St.<br />Activity & Recreation Center (ARC), 1701 W. Ash St.<br /><br />The survey closes on April 21. Participation is voluntary and responses are anonymous. Both organizations thank the respondents in adv
