 request by Osama Yanis (owner) to annex 15.37 acres of land into the 
city, applying R-1 (One-Family Dwelling) zoning on 11.75 acres and PUD 
(Planned Unit Development) zoning on 3.62 acres; and approving a 
development plan to be known as "Forum Blvd PUD Plan".  The subject site
 is located on the east side of Forum Boulevard, approximately 350 feet
