OPEN ENROLLMENT for health, dental and vision insurance.
 
Now is the time to add or remove dependents for insurance coverage or change insurance plans.
 
If you need to make a change to your insurance, which will be effective January 1, 2012, please complete an open enrollment form (attached to this e-mail) and return it to Human Resources by November 18, 2011.
 
As a reminder, the $750 co-pay plan will have bi-weekly premiums deducted for employee only coverage starting in January.
 
If you do not need to make any changes to health, dental or vision coverage, you do not need to complete an open enrollment form.
