Birth certificates and immunizations required for kindergarten registration
(COLUMBIA, MO) - Columbia Public Schools kindergarten enrollment for the 2005-06 school year begins in April 2005. The Columbia/Boone County Health Department offers immunizations and issues certified copies of birth certificates for children who are registering for kindergarten. In an effort to simplify the process for parents, we offer the following options to obtain birth certificates:
* Go to our Web site at http://www.GoColumbiaMo.com/Health/birth_certificates.html and click on "Birth and Death Certificates" to print out a birth certificate application.
* Mail the application and a $15 check or money order to the Health Department at P.O. Box 6015, Columbia, MO 65205. The birth certificate will arrive in the mail in approximately one week.
* Or, bring the filled out form, $15 (cash or check) and a photo I.D. to the Health Department.
* Or, go online to www.vitalchek.com and order a birth certificate with a major credit card.
* Or, call 877-817-7363, and for $24.95 order a birth certificate over the phone with a major credit card (for an extra charge you can have it expedited).
If you do not have Internet access, an application can be picked up at the Health Department's Registrar Office. The office is located at 1005 West Worley and is open Monday-Friday 8 a.m. to 5 p.m. and until 6 p.m. on Tuesdays.
The Health Department can only provide certified copies of Missouri birth certificates. To obtain a certified copy of a birth certificate for a birth outside Missouri, visit www.vitalchek.com.
The Health Department also offers all immunizations required for enrollment in Columbia Public Schools on a walk-in basis Monday-Friday 8:30 a.m. to 4 p.m. For a childhood immunizations schedule and cost information go to www.GoColumbiaMo.com/Health/Clinic_and_Nursing/ or call (573) 874-7356.
