The press release issued on June 28, 2010 attributed the incident at the bio-chemistry lab to human error. 

Columbia Fire Department investigators and University of Missouri officials have not concluded the investigation and hence the cause of the explosion at the laboratory is unknown. 

Human error has not been positively identified as the cause or a contributing factor.  

The Columbia Fire Department regrets the release of incomplete information and is taking steps to ensure accuracy in future incidents.
