Columbia/Boone County Public Health and Human Services Seeking Community Input with Boone County Community Health Survey

COLUMBIA, MO (June 12, 2013) - The Columbia/Boone County Department of Public Health and Human Services is encouraging all citizens of Boone County to make their voices heard and take the Boone County Community Health Survey.

 

The survey is being distributed as part of the department’s MAPP (Mobilizing for Action through Planning and Partnerships) process, which is a community-wide strategic planning process for improving public health.  MAPP helps communities prioritize their public health issues, identify resources for addressing them and implement strategies to improve health.

 

The survey results will be utilized to identify the most pressing public health issues facing our community which can be addressed through community action.

 

The survey can be found at: www.gocolumbiamo.com/Health or through the department’s Facebook page: www.facebook.com/CoMoHealthDept

 

For those who don’t have online access, paper surveys may be picked up at the Columbia/Boone County Department of Public Health and Human Services, 1005 West Worley, in Columbia.

 

The survey ends June 30th, so don’t wait!  Your opinion is important to us; the survey is short and only takes a few minutes to complete; and you could win a $25 gift card just for telling us what you think about health issues in Boone County.
