*Input on the Annual Arts Funding Process*

Survey forms were sent to applicants along with information on their scores
and preliminary award amounts. Please complete the survey and return it to
OCA as soon as possible so we can continue to assess and improve our
funding process. Your input is important to us as it helps us to assess the
clarity of the application form and the overall ease of the process.
